Welcome to our center for application forms and instructions. We will be adding to this page in the future, but have the following forms available for you to download now. After printing out the form and completing it, you can fax it to us, mail it in or hand deliver it.

| Notice Inviting Qualified Bidders | The City of Brea has issued its 2013 Notice Inviting Qualified Bidders for contractors wanting to participate in the City's informal bid process. | Non-Injury Traffic Collision Report Form | Use this form if you have been involved in a non-injury traffic collision on private property. Directions on how to complete this form are included. | |
| Overnight Parking Permit Short-term, Temporary |
Annual parking permits are intended to relieve parking situations. A permit may be obtained for any operable vehicle that needs to be parked overnight on the street in front of the residence. The short-term, temporary placard can be self-issued for an overnight guest. |

| Report Release Form | Use this form and directions sheet to obtain a police report. Fax, mail or drop off your form at the Brea Police Department. Copies of arrest reports must be obtained via the North Justice Center Disrict Attorney's Office. | |
